Counseling Center – The Center for Self-Actualization
The Center for Self-Actualization operates two locations in the Chicago metropolitan area: Evergreen Park at 3317 W. 95th Street, Suite 101, IL 60805, and Blue Island at 13116 S Western, 2nd Floor, IL 60406.
